REQUISITION FORM NOTICES:
  1. All Lab tests ordered should be medically necessary and reasonable for the diagnosis or treatment for a Medicare or Medicaid Patient.  The office of Inspector General takes the position that a person who orders or influences the ordering of non-medically necessary test which Medicare and Medicaid reimbursement is claimed may be subjected to civil penalties under the False Claims Act.
  2. The full Comprehensive Panel includes all tests listed in the table above.
  3. Metabolites will be automatically confirmed when affiliated parent drugs are marked as requested tests on the requisition form.
  4. Tests listed with an asterisk (*) on the requisition form are only available for urine samples. Validity testing will only and automatically be conducted on all urine samples
  5. When ordering, the test panel will default to qualitative screening and confirm positives and prescriptions, unless otherwise specified.
  6. Qualitative Screen will be performed on the following tests if ordered: Cocaine, THC, Amphetamines, Opiates, Oxycodone, Benzodiazepines, Barbiturates, Methadone, PCP, and EtG/EtS (Alcohol)
  7. To see the most current list of tests on the Comprehensive Panel, reference the table above.
  8. The diagnosis codes found on the requisition form are listed as a convenience only. Physicians should use the ICD-10 code that best describes the reason for ordering the test, whether or not that code is listed below.
